The challenge
National HIV programs run hundreds of POC diagnostic devices across remote facilities with intermittent connectivity. Programs need real-time visibility into fleet operations, secure transmission of diagnostic results into national health information systems, and the ability to manage devices remotely — without sending engineers to the field for every issue.
What we built
Encrypted connectivity from each device to its national DHIS2 instance, with each country's data fully sovereign. Real-time fleet visibility across 2,000+ instruments. Firmware and configuration updated remotely, no field visits required. Consumables tracking built alongside results, making supply chain forecastable rather than reactive. A single operational view across all eight countries.
What it's teaching us
- Store-and-forward over unreliable mobile networks is the foundation, not an afterthought.
- Consumables visibility changes program economics more than results visibility does.
- Segregated national instances aren't a workaround — they're the right architecture for multi-country sovereignty.
